Asian Pacific Development Center’s (APDC) Integrated Care Services is a blending of our existing services with primary care medical services to create a coordinated, comprehensive system of assistance for APDC’s clients. While intended to serve the Asian American Pacific Islander (AAPI) communities in general, Integrated Care Services will target the indigent, refugees and first-generation immigrants. The intake process includes the development of a comprehensive action plan for the individual and their families, i.e., primary care, behavioral health, wellness, ESL classes, youth services. Services are then seamless provided directly by the agency or when necessary, via referral (with assistance of a navigator). For example, should a doctor identify a potential patient issue with depression, an immediate intervention can be arranged as the behavioral health clinician is officed in the same building. The seamless integration, there will be a merging of medical care, behavioral health and social programs.
